My health for life is a free, six month program where you work with a health coach to achieve your health goals.
It is a State government-funded initiative, delivered by an alliance of health organisations, and is designed to help Queenslanders stay well and lessen their risk of developing conditions such as type 2 diabetes, heart disease, stroke, high cholesterol and high blood pressure.
As a program participant you receive six sessions of health coaching and support over six months, by way of phone coaching or in small group sessions. Assisted by your coach, you will explore topics including nutrition, physical activity, understanding your personal risk of developing health conditions, and how to make healthier choices a part of everyday life.
You also develop an action plan to help you achieve your health goals. Whether your goal is to lose weight, get more active, eat better, or stress less, your plan will be tailored to your own lifestyle and the healthy change you want to make.
Telephone coaching is a more flexible option, allowing you to participate in the program at a time and location that works for you.
You will have six appointments with your health coach over six months. Appoointments one to five are fortnightly and you will have a 12-week break between sessions five and six.
Group programs are available across Queensland and are delivered by local health professionals in your community. They offer a positive and supportive environment to share and learn as a group, while working on your own personal health goals.

Our programs for Culturally and Linguistically Diverse Communities (CALD) are delivered by The Healthier Queensland Alliance partner Ethnic Communities Council of Queensland (ECCQ). All CALD programs are delivered by qualified, bilingual multicultural health workers and are tailored to meet the needs of our participants.
